The Risks of Smoking Before Knee Replacement

Smoking before knee replacement surgery can have negative effects on the outcome of the surgery and is associated with an increased risk of complications. Smoking can cause:

  1. Increased Risk of Joint Infection
    • Smoking is associated with an increased risk of knee infection after a knee replacement. A joint infection can be a disaster as it may require implant removal and prolonged IV antibiotics.
  2. Increased Wound Complications
    • Smoking delays wound (incision) healing, which can the lead to a joint infection (#1 above). Nicotine constricts blood vessels, reducing blow flow to the surgical site, which then delays or impairs wound healing.
  3. Increased Anesthesia Risk
    • Smoking increases the risk of cardio-pulmonary complications during surgery.
  4. Increased Risk of Blood Clots
    • Smoking is associated to an elevated risk of deep vein thrombosis (DVT), commonly referred to as blood clots. The severity of this issue heightens when blood clots travel to the lungs, a condition known as Pulmonary Embolism. Timely treatment is crucial, as Pulmonary Embolism can have severe consequences if not addressed promptly.
  5. Increased Pain and Slower Recovery
    • Smokers can experience more pain after surgery and may have a longer recovery.
  6. Worse Overall Outcomes
    • Studies have shown that smokers generally have worse outcomes after total knee arthroplasty. This includes lower satisfaction rates, less improvement in joint function, and a higher likelihood of needing revision surgery.

Dr. El-Yussif’s Recommendations:
I recommend that patient’s quit smoking for at least 6 weeks prior to surgery. Smoking cessation can significantly reduce Postop complications and improve patient satisfaction.
